Transition Destinations
- Acute Inpatient Rehabilitation (AIR): ≥3 hours therapy/day tolerance, medically stable, motivated participation
- Long-Term Acute Care (LTACH): Ongoing ventilator weaning, complex wound care, or medical needs exceeding AIR thresholds
- Skilled Nursing Facility (SNF): Lower therapy intensity needs with continued skilled nursing support
